Membership Instructions 2006
Can you believe it! We have instructions on how to fill out all the membership forms. We are not insane, yet. But with National membership requirements and the many releases and information required, a set of instructions seemed logical. Plus, we have tried to put everything we could possibly need from you for the whole year in this package.
So please get yourself a good cup of coffee, have a seat and smile and think I only have to do this once a year!
For your convenience the blue links will either launch a PDF file or link you directly to U.S. Pony Club web site form section.
1. 2006 Membership Report Form (click here) (3)
- We need 1 original and 2 copies.
- This form is available on-line at www.ponyclub.org in an interactive version and can be filled in on-line. Leave the top portion blank, fill in from “Member Information” down.
- Member and one parent/guardian need to sign both pages.
- If the form is returned for corrections, the USPC charges $5.00
2. 2006 USPC Medical Release Form (click here) (5)
- We need 5 forms, all with original signatures; preferably in blue ink, dated.
- This form is available on line at www.ponyclub.org.
3. USPC Competitior’s Activity and Rally Release (click here) (3)
- We need 3.
- We need original signatures on all 3 Releases but leave the date blank.
- Fill in sections relating to the member and pony only; leave sections relating to kind or type of Rally blank.
- If you are using a pony not owned by you, that owner must sign also.
4. Uniform Chaperone Rules/Duties (click here) (2)
- For parents.
- We need 2, both with original signatures, leave the date blank.
5. Deep Run Hunt Club Equine Activity Liability Release (click here) (2)
- We need 2; original signatures on both, dated.
- This form is required even if you are a Hunt Club member.
6. Morven Park Release (click here) (1)
- Original Signature; leave the date blank.
7. Coggins for EACH mount used in Pony Club activities. (5)
- We need 5 copies of your current (drawn within one year of now!) Coggins.
- Put the member’s name and name of mount on upper right corner.
- Please remember to up-date your Coggins with us when you get a new one.
7. A check made payable to Deep Run Hunt Pony Club in the amount of $145.00.
- FYI: $80 is paid to USPC for your membership and insurance coverage and $20 is paid to the USPC Virginia Region to support the Region’s activities.
